New patient paperwork is available by clicking the appropriate link below. Please PRINT the packet and complete the information requested. Please note that your information will not be directly entered into our system. If you are unable to print the paperwork, please arrive 15-20 minutes early and request a new patient packet from our front desk.
Please bring your completed and signed paperwork to your first appointment.
Please arrive on time for your appointment. Due to the nature of our procedures, we require 30-45 minutes for each patient. We make every effort to stay on schedule to respect your time and appreciate the same consideration.
When you come for your appointments, please remember to bring the following:
- Driver's License or a valid ID
- Insurance information
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans etc. and any other relevant information
- List of medications (if any)
Office Policies
Treatments: By appointment only. Please call our office at 817-416-0970 to schedule an appointment.
Fees: We are now accepting most major medical insurances.
Appropriate attire for your visit: Please wear loose fitting clothes, such as athletic clothes, to allow for full range of motion. If your appointment entails an injection-based procedure, please wear clothing allowing access to the affected areas.
Cancelations: Must be made 24 business hours in advance to avoid a cancelation charge.
